Contacting by Phone

Call 1-888-INFO-FDA (1-888-463-6332). This is the main phone number for the FDA, and most questions can be answered by calling this number.

Call 1-866-300-4374 to report an emergency. If you believe that you have found a dangerous product, call this number to report it to the FDA. If you have a life-threatening medical emergency, call 911 and get emergency help first.

Call 1-800-332-1088 to report a fraudulent or illegal medical product. If you found a drug or medical product that is being sold illegally, or violating the law, call this number to report it.

Call 301-796-3900 for questions related to Freedom of Information Act (FOIA) queries. Use this phone number to make a FOIA request or to follow up on one.

Call 301-796-4540 for press inquiries. If you are a member of the media, you can call this phone number to ask questions related to a story that you are writing.

Using Online Forms

Use the Safety Reporting Portal to report safety issues in products that the FDA regulates. You can use this form to report issues with the following products: Drugs Food Animal Food Tobacco Products Dietary Supplements
