Tumors can be benign or malignant; the latter is cancerous and invades nearby tissues, facilitating metastasis— the spread to distant body parts through blood or lymphatic systems. As cancer progresses, cells evade the body’s control mechanisms, disrupting normal functions. Understanding the intricate molecular and cellular processes driving cancer is crucial for developing effective prevention, diagnosis, and treatment strategies.

Dr Pooja Babbar, Consultant, Medical Oncology, CK Birla Hospital, Gurugram, says, “Due to the fact that millions of individuals globally suffer from cancer, numerous studies have been conducted to determine the origins, course, and dissemination of the disease within the human body. The current study investigates the intricate pathways by which cancer starts, grows, and penetrates a human body.”

The initiation of cancer can be traced back to genetic mutations occurring within normal cells. These alterations in the cell’s DNA disrupt the usual mechanisms that regulate growth, leading to uncontrolled cell division. This unchecked proliferation culminates in the formation of a mass known as a tumor. The tumor, initially confined to its site of origin, can exhibit varied characteristics and behaviors depending on the specific type of cancer.

“As the tumor expands, it undergoes changes that may grant it the ability to invade adjacent tissues. The invasive nature of cancer cells means they can breach normal boundaries and infiltrate surrounding structures. This invasive potential is a critical hallmark as it sets the stage for the most concerning aspect of cancer—metastasis,” says Dr Vikas Goswami, Director, Medical Oncology, Max Super Speciality Hospital, Vaishali.

Metastasis is the process by which cancer cells break away from the primary tumor, enter the bloodstream or lymphatic system, and migrate to other parts of the body. These disseminated cancer cells can establish secondary tumors in distant organs or tissues. Metastasis represents a pivotal point in cancer progression, significantly complicating treatment and prognosis.

Understanding the sequential stages of cancer development – from genetic mutations to uncontrolled proliferation, local invasion, and eventual metastasis – is paramount for effective management. “Early detection becomes crucial in the fight against cancer, as interventions at the initial stages offer a better chance of success. Moreover, comprehending the specific characteristics of a cancer, including its potential for metastasis, aids in tailoring targeted treatments that address the unique features of the disease,” adds Dr Goswami.

Dr Babbar, “A change in a cell’s DNA is necessary for the development of cancer. A mutation is a change that can be brought about by a number of different things. These alterations can be brought on by outside variables including exposure to chemicals, UV light, or carcinogens like tobacco smoke. Internal variables can also have a role in the development of cancer, such as genetic abnormalities acquired from parents. The cell is able to divide and grow uncontrollably because of this genetic modification. The body’s cells normally follow a controlled cycle of growth and division, stopping when needed to avoid undue tissue expansion. On the other hand, cancer cells resist this control, continuing to grow until they build up into a tumour.”

Furthermore, cancer can spread via the lymphatic or circulatory systems to other areas of the body. Certain malignancies, such as leukaemia, begin in the blood cells and then spread to other parts of the body.
